ezboutique on "[Plugin: WooCommerce Checkout Manager] ERROR: Username Required"

I installed on the appply template and added 4 custom fields. After adding the fields when I go through checkout I get an error:

Error: username required

If I disable the checkout manager, the checkout works just fine, so the error is related to the plugin. Any suggestions?
