I have a problem two days ago. Sometimes, my site duplicate media in the library giving a name as "rp_Baix%2520la%2520mar%2520inf.jpg". Also, that text appers instead the picture, and no author shows. There is no action from me what generates it, I suppose. I don't know if a plugin or the Wordpress update creates the problem.
